Step 2: 28-Day Posting Period
After the health care provider submits the Description of Services Requested and Certification Form (FCC Form 465), USAC reviews it to ensure program compliance and posts the completed Form 465 on its website.
The posted FCC Form 465 provides information about the HCP and its need for services to service providers that might wish to bid to provide the services.
Each HCP's FCC Form 465 must be posted on the USAC website for at least 28 days prior to the health care provider selecting a service provider in order to fulfill the program's competitive bidding requirement.
HCPs may enter into agreements to purchase services after 28 days have elapsed (i.e. on the 29th day), and after an FCC Form 465 was posted on the USAC website. Entering into any agreement during the 28-day posting period is prohibited.
USAC will send each new applicant a "Receipt Acknowledgement Letter" confirming that its FCC Form 465 was posted on the USAC website. Returning applicants using e-certification will receive a confirmation by email. The confirmation of posting sent by USAC will indicate the date on which the HCP may enter into an agreement to purchase services from a service provider. This date is known as the Allowable Contract Selection Date (ACSD).
